What will you be selling, offering, and or displaying during your class and at your booth? Please include the brand names of products. * If you add the same brand name product that someone else is already selling, you will be contacted and asked to take it off. This is a first-come basis. *

If you do not put it on this form, you will need to verify that no one else has this item before bringing it. You will also need to contact Tammy to add to your list.

I agree to not burn incense or candles, diffuse oils, etc. as some may trigger migraines for some attendees. We also do not allow drumming, loud instruments or PAs at the event. This can be disturbing to vendors around you.

I understand that Hope Haven Events nor the facility is not responsible for lost or stolen items.

We try our best not to have the same company product at more than one booth. Please know we are human, and sometimes mistakes do happen. If this does happen, we will not refund you but, do our best to see if there is any solution. This might include asking you not to sell or use this product at the conference. We ask everyone to please be kind and professional about it.

STRICT POLICY ON LEGAL COMPLIANCE. Please read carefully before you agree. This is a legal binding agreement.

This is very important. Please read carefully. Legal Compliance: It is important to know the laws in your area as well as the conference area. It is important to understand that they will affect you if you break the law. Any vendor who violates a compliance to the local laws will be asked to leave immediately and no refunds issued. It's pretty common knowledge, but here's the rundown: -Do not make any claims, guarantees, cures or imply any of them. -Do not diagnose or claim to treat any disease or illness of the body in any of its facets unless you have the legal license to do so. -Do not tell people to stop their medication unless you are their primary physician and have the legal authority to do so. -Do not claim to tell the future or imply to it. -Do not tell someone you believe they are dying or that someone close to them is dying. *This is not a religious event in any way. No proselyting allowed. Please act in a universal way that would allow people of any belief system to feel accepted here. Thank you for allowing us to continue offering these high caliber events by agreeing to legal compliance. *

I agree to fill a minimum of 7 general session tickets. The means to fill those tickets may include: your support team, volunteers, staff, employees, downline, friends, family or preferred clients. We would love (if possible) for some of those tickets to go out to social media giveaways, contests or to your email list. Do you agree to promote the event and do your part by filling at least 7 free tickets?

Hope Haven Events is required to send a list of all vendors with their Name, Address, Phone Number, Tax EIN# or DOB & Driver's license # with State issued, and indicate if they are or not selling to the Tax Commission. To make sure we are giving them the correct info, we are asking the following five questions. In addition, you will receive a tax paper to send to them for this event at registration. This paper does not go to us. You will need to mail it to the Utah Tax Commission by the stated deadline.
